Purpose
The Listing Form is the rollup of the household Survey. Each parish CERT team or NDMD field officer submits one Listing per affected sector, summarising the total damage, totals affected, and immediate aggregate needs.
Before you startHave these ready
- All completed household Surveys for the affected sector
- Sector boundaries (street, district, parish)
- Estimates for the number of households affected at three severity tiers
- Public infrastructure damage (roads, water, electrical)
- Coordination contact for follow-up

The Initial Damage Assessment Listing Form will be embedded from the forms plugin once it's imported. Until then this page describes the form, what you'll need to complete it, and how it fits into NDMD's wider operations.
