Initial Damage Assessment Listing Form

The aggregated parish-level listing that rolls up the household-level Damage & Needs Survey into a single record per affected area — used by NDMD and partner agencies for situation reports.

What this form is for

Purpose

The Listing Form is the rollup of the household Survey. Each parish CERT team or NDMD field officer submits one Listing per affected sector, summarising the total damage, totals affected, and immediate aggregate needs.

Before you start

Have these ready

  • All completed household Surveys for the affected sector
  • Sector boundaries (street, district, parish)
  • Estimates for the number of households affected at three severity tiers
  • Public infrastructure damage (roads, water, electrical)
  • Coordination contact for follow-up
