Initial Damage & Needs Assessment Survey

Standard household-level Initial Damage & Needs Assessment Survey used after a hazard event — collected by parish CERT teams or NDMD staff to feed the national post-event picture.

What this form is for

Purpose

After hurricanes, floods, earthquakes or other events, NDMD coordinates a federation-wide Initial Damage Assessment (IDA). This survey is the per-household instrument used to capture immediate damage, needs and welfare information.

Before you start

Have these ready

  • Household address and parish
  • Number of occupants, including children and persons with disabilities
  • Photographs of structural damage (if safe to capture)
  • Current shelter status — at home, with family, in public shelter
  • Immediate needs — water, food, medical, building materials
